PATIENT INFORMATION

Referral Policy

All patients must be referred by their General Practitioner, Optometrist or Specialist. Referrals will remain current for 12 months from a General Practitioner and Optometrist or 3 months from a Specialist.

 

It is the patients responsibility to have a current referral at their appointment.

 

Cancellation Policy

If you need to cancel your appointment please let us know as soon as possible either by phone or email.

 

Appointment Reminder

Please be aware we don't remind you if you are due for your yearly check up, this is your responsibility to have a new referral at the office 3-6 months before you are due so we are able to book your appointment in the correct month.

 

Appointments

Consultation time may vary and can take anywhere up to 90 minutes (depending on the type of tests conducted).  Unfortunately sometimes we do run late. On the day of consultation, if you can’t wait longer, you can approach the reception for re-scheduling.

 

Your eyes may be dilated at your appointment so please be aware you will need a driver to take you home after your appointment as you will not be able to drive.

 

Please bring a pair of sunglasses to your appointment to wear afterwards.

 

Time is wasted each week due to patients not advising us in advance that they will not be attending their appointment. These are valuable appointments that could be given to someone else. We ask that you contact the clinic as soon as you receive your appointment to confirm you will attend. 

 

If you feel your condition is deteriorating or you feel you need to be seen sooner than you’re allocated appointment, please contact your referring GP or optometrist.

 

Please note that we are NOT a Bulk Billing Practice and we require payment IN FULL on the day of your appointment.  This includes payment for your consultation and any extra investigations.

 

Treating Practice Staff with Respect

We value all of our staff who work very hard to provide you with a first class service. Please treat them with the same respect you would expect yourself.

 

Smoke-Free Buildings

The practice operates a smoke-free policy. Smoking is not allowed in any part of the practice premises.
